David P. Cook
Graveside services for David Paul Cook, 41, Lawrence, will be at 11 a.m. Thursday at Eudora City Cemetery.
Mr. Cook died Tuesday, April 11, 2006, at Midland Hospice House in Topeka.
He was born June 7, 1964, in Long Beach, Calif., the son of Paul Dayton and Jo Ann Ford Cook.
Mr. Cook graduated from Eudora High School in 1982.
He served in the United States Navy from 1982 to 1985. He later attended Kansas University and graduated in 1997 with a degree in social welfare.
Mr. Cook was a member of Trinity Episcopal Church.
He married Dawn Woodside. They divorced.
Survivors include his mother, Jo Ann Cook, Mons, Belgium; two brothers, Robert Cook, Fort Scott, and Jeffrey Cook, Eudora; and two sisters, Cynthia Hemberg, Mons, Belgium, and Stephanie Miller, Eudora.
His father, Paul Dayton Cook, and a brother, Christopher Patrick Cook, preceded him in death.
Memorials may be made to Midland Hospice House, 200 S.W. Frazier Circle, Topeka 66606.
